Application Information

If you have decided to apply for either the PLC or OCC program, the application paperwork that you need is linked below. Please follow the instructions in detail.

Rough Application
This document is the basic information that we need to begin your file and prepare for submission of a more formal application to the selection board. Print, read all instructions, complete, and return to our office via mail or fax. Use the checklist included on Page 3 to insure you gather all required documents and submit all required forms.

Academic Certification Form (ACF)
Print, fill out the top blocks (Name, SSN, School, and sign) and give to your school registrar. They need to fill out the bottom portion of the ACF and return to us via mail along with your official transcripts. We need this form and transcripts from each college/university you have attended, so print additional forms as necessary.

680 - Request for Exam
This form is used to schedule your physical when we are ready for that step. Fill out blocks 1 - 15 and blocks 26 - 29. Print very neatly. Sign blocks 21 and 25.f. Return to our office with your Rough Application.

2807 - Medical History Report
This form is used in conjunction with the 680 to schedule your physical. MEPS (Military Entrance Processing Station) will review your medical history and request any additional medical records as necessary. Complete block 1 a-e and answer each of the 75 questions by placing your INITIALS in the appropriate answer column. Any YES answers must be explained in detail in the space provided. Detail includes the medical issue, your doctor, and when it occurred. Understand that each YES answer will complicate your physical process and may require medical documentation. Complete blocks 3 - 6 on page 4 and sign block 7. Return to our office with your Rough Application.

Dean Personal Information Questionnaire (PIQ)
This is a reference from the Dean of your school (or department/students, etc.). Please print, fill in your name and sign in the appropriate block, and give (or email) to your dean for completion. We ask that your Dean return this directly to our office via either mail or fax.

This is the reference form for your other references (employer, professors, etc.). Please print, fill in your name and sign in the appropriate block, and provide to them for completion and return to our office via either mail or fax.

National Agency Check (Background Check)
FOR SENIORS AND GRADUATES ONLY. Access this link and open SF form 86. Follow the instructions and return to our office.
